NOTE - We have tightened the requirement that products you bring contain locally grown ingredients. All value-added products sold at the winter farmers market must include a primary ingredient that is locally grown or raised.
(This Permit is required for Vendors selling Value-Added Products.)

The Town of Greenfield's Temporary Health Permit is also called the "Temporary Food Service Application Form".
- Please complete the entire form except for the parts we have noted below.
- The permit fee is $15(not $30 as indicated on the form).
- #11 on Page 2 is NOT REQUIRED.i.e. the sketch on the 4th page is NOT required.
- #12 on Page 2: Click on the link to view and print the "Are You Ready" Checklist.
- Sign and Date the form on Page 2 and Page 3.
- Make your Check payable to the "Town of Greenfield".
- Mail the completed Permit (with any supporting documents required for the Permit) and $15 check with the Vendor Application and $15 fee to Danya Teitelbaum, 244 North St., Northampton, MA 01060.
